How to Submit

To submit your Proof of Immunization Compliance go to the Student Health Web Portal. You will then go to Required Immunizations and  Forms to upload your proof of vaccinations into the system. You can also send by fax or email. If you have questions about your immunization requirements please call (251) 460-7151 or email immunizations@southalabama.edu
