Remote Patient Monitoring (RMEDE™)
The Realtime Medical Electronic Data Exchange System (RMEDE™), developed by the Center for Strategic Health Innovation (CSHI), is a secure, cloud-based, life-state management system that enables in-home monitoring of patients with chronic disease, as well as interventional health informatics reporting. RMEDE™ adheres to NIST 800-53 framework.
The RMEDE™ Remote Patient Monitoring (RPM) Program is used to track physiological parameters of patients diagnosed with chronic diseases and report non-compliant ranges to healthcare providers in order to improve clinical outcomes. Benefits include:

CSHI continues to enhance and support the RMEDE™ program. There are currently over 1,000 active Medicaid patients participating in our remote patient monitoring program.
Features of this system include:
- Uses Interactive Voice Response (IVR) technology

- Detects threshold violation readings for blood pressure, blood glucose and weight
- Generates threshold violation reports in real time and/or periodic graphs
- Provides link to Alabama Department of Public Health (ADPH) (RPM) Nurse for patients enrolled in the Medicaid remote patient monitoring (RPM) program
- Monitors Congestive Heart Failure, Hypertension, Diabetes, and Maternity Care for Medicaid patients
Referral: ADPH (RPM) Nurse receives the referral and provider-set data limits for the monitoring measures
Enrollment: ADPH (RPM) Nurse completes a home visit with the patient to place equipment and provide
instruction
(CSHI provides required monitoring software and medical equipment as needed)
Alerts: Readings outside of provider-set parameters produce a threshold violation and an alert is automatically sent via e-mail and/or text message from RMEDE™ to the ADPH (RPM) Nurse. The nurse then follows up as needed; making contact with the patient, evaluating what action is needed and providing education, reassurance and/or referral. Reports can be faxed to the provider.
Reports: Periodic graph reports of patient data are sent to the Provider Ongoing Contact; ADPH (RPM) Nurse makes contact with patient at least monthly. Additional Data Analytics are provided via Tableau®.
Renewal: Provider orders are renewed yearly
